EvexiPEL® hormone replacement therapy
EvexiPEL® is a bioidentical hormone replacement therapy for women and men. Custom, plant based pellets keep hormone levels steady, which can ease fatigue, weight changes, low libido, sleep trouble, and mood shifts. Pellets are placed in a quick in office visit with a local anesthetic and little to no downtime.
How long pellets last depends on your sex and your metabolism. Women typically return every 3 to 4 months, and men every 5 to 6 months. Follow up labs help us fine tune each new dose.
Pellets are our specialty, and your plan can include more than pellets. Following the EVEXIAS approach, many patients take prescription progesterone as a nightly oral capsule alongside their pellets, which can also support sleep. We also offer injections when another route fits your body or lifestyle better, and HRT Complete (E) or (T) supplements to help your body use its hormones.

Insurance usually covers only what it considers medically necessary. Our flat rate cash panels look at your health from a preventative standpoint, so we can check the full picture instead of a few markers at a time.

Decades of experience, a proactive approach
Appell & Sorrell Wellness is a medical clinic in Vestavia, near Life Time Fitness, specializing in hormone and metabolic health. We are led by longtime Birmingham residents and orthopedic surgeons Dr. Scott Appell and Dr. Robert Sorrell. After decades of helping patients improve recovery and mobility, they created a place for integrative care focused on prevention and longevity.
Joined by nurse practitioner Ashley Whitsett, we help patients feel better and live brighter every day.
Scott Appell, MD
Board certified orthopedic surgeon
A longtime Birmingham resident specializing in total joint replacement and intervention, with more than four decades in the field.
After earning his bachelor’s degree at the University of Florida, Dr. Appell attended the University of Florida College of Medicine and completed his residency at the UAB Heersink School of Medicine. He has long incorporated weight loss plans with orthopedic patients to promote recovery and mobility, and spent seven years with the former Complete Weight Loss and Healthcare.
Robert Sorrell, MD
Board certified orthopedic surgeon
A lifelong Birmingham and Vestavia Hills resident specializing in sports medicine, joint replacement, and arthroscopic surgery.
Dr. Sorrell is an Auburn University graduate and was an Academic All American and All SEC baseball player. He attended medical school and completed residency at the UAB Heersink School of Medicine, and has been in private practice in Birmingham since 1992, helping patients with weight loss to improve surgical results, recovery time, and mobility.
Ashley Whitsett, CRNP
Family nurse practitioner
A lifetime Birmingham resident with extensive clinical training in weight loss medicine.
Ashley earned her bachelor’s degree from the Auburn University College of Nursing. While working in the Pediatric Intensive Care Unit at Children’s of Alabama, she completed her family nurse practitioner degree at the UAB School of Nursing, where her clinical hours focused on weight loss medicine.

What happens at my initial consultation?
Your first visit is a 30 minute consultation with one of our providers. We go through a detailed medical history, review your symptoms and how long you have had them, and talk about your goals. From there, your provider recommends the right labs and next steps. The initial consultation is $125.
Why do you offer cash labs at a flat rate?
Insurance usually approves only the tests it considers medically necessary. Functional medicine looks at testing from a preventative standpoint, and paying cash lets us order complete panels covering hormones, nutrients, and metabolic markers so we see the full picture.
Can I use my HSA or FSA?
Yes. Appell & Sorrell Wellness is HSA and FSA eligible, so you can use those funds toward your visits, labs, and treatment.
Can I share my labs with my primary care provider?
Yes. We specialize in hormone and metabolic care, and we always encourage you to keep your primary care provider in the loop.
Does my GLP-1 price change if my dose goes up?
No. Your monthly fee stays the same at any dose: $350 for semaglutide or $450 for tirzepatide, with prescriptions delivered to your door.
What happens during a pellet insertion?
It is a quick in office procedure using a local anesthetic, with little to no downtime. Pellets typically last 3 to 4 months for women and 5 to 6 months for men, since sex and metabolism both affect how quickly your body uses them.
How much do pellets cost?
Pellet insertion is $330 for women and $770 for men. Before your first insertion you will have comprehensive labs ($250) and an initial evaluation ($125). Follow up labs and check ins are $75 and help us adjust each dose. We accept HSA and FSA.
Do you offer hormone therapy besides pellets?

EVEXIAS HRT Complete (E) and (T) supplements can support how your body uses estrogen or testosterone. Your provider will help you choose based on your labs, your body, and your routine.
Who is a candidate for hormone therapy?
Women and men with signs of hormone imbalance, like low energy, mood changes, reduced libido, sleep problems, or weight gain, may benefit. A consultation and lab work are required to know for sure.
Do you treat men too?
Yes. EvexiPEL® is customized for both women and men, and our men’s quiz is a good place to see whether testosterone may be part of what you are feeling.
